Antibiotic Stewardship
The overuse or misuse of antimicrobials medication is a serious public health issue. In order to improve the use of these medications, Sunnyside has assembled an Antimicrobial Stewardship Program.
Our program promotes the appropriate use of antibiotics for the purpose of:
- Improving patient outcomes
- Reducing antibiotic resistance
- Decreasing the spread of infections caused by multi-drug resistant organisms
- Tracking the use of antibiotics and resistance patterns within our facility
- Sharing information on antibiotic use and resistance to doctors, nurses and relevant staff
- Education for clinicians about resistance for best practice
- Reduce hospital readmissions to the acute hospital
Diabetes Management
Our nurses provide help with all aspects of managing diabetes including:
- Blood glucose monitoring
- Insulin administration and tracking
- Exercise planning
- Menu planning, education and more

R.E.A.C.H. your recovery goals with Sunnyside Nursing and Post-Acute Care’s interdisciplinary approach to stroke recovery. Our patient oriented individualized rehabilitation program complements our nursing care to offer optimal recovery after a stroke. We work closely with our patients, family, physicians, and nursing team to achieve mobility, build strength, and improve balance. Our team focuses on improving speech and swallowing and to enhance skills needed to perform activities of daily living. Our program includes:
- Neuromuscular reeducation
- VitalStim (r)
- The Lee Silverman Voice Treatment programs LSVT BIG (r) and LSVT LOUD (r)
- Other programs and techniques to assisted in recovery
Restorative Nursing Program
At Sunnyside Nursing and Post-Acute Care we seek to maximize the quality of life for our long-term care population, who require a specialized restorative program.
Our therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:
- Ambulation
- Range of motion
- Activities of daily living
- Positioning and alignment
- Skin integrity and wound management
- Contracture management
- Bowel and bladder training
- Rehabilitative and social dining
- Strengthening and restraint reduction
- Weekly and monthly height and weight monitoring
- Antibiotic stewardship program
Dietary Program
To ensure that each resident is receiving the proper nutrition we offer:
- Meals to meet specific dietary and therapeutic needs, according to physician orders
- Regular nutritional assessments from our registered dietitian (RD)
- Clinical nutrition doctor who works with our RD to ensure each resident receives optimal nutrition
